This 22 minute program with links, will explain the legal requirement for all legal and accounting firms, with one or more employees, and provide a concise plan for compliance, including model forms. All California employers are required to establish a safety plan and emergency plans and our Association has developed this program to help members comply in an easy and clear way.

Mandated Forms
State regulations mandate employers to provide certain information to their employees. State Fund provides this information in the form of posters and brochures as a service to our members. Employers are required to distribute the information to workers in the following instances.
- When an employee is injured, you must provide them a Workers’ Compensation Claim Form, DWC 1: http://www.scif.com/pdf/e3301.pdf
- Also, when an injury occurs, you need to report it immediately to the State Fund. You can call State Fund, 24-hour Claims Reporting Center toll free at (888)222-3211 to file your report, or fax your injury report to (800)371-5905, using the following report:

- Using the State Fund’s Medical Provider Network (MPN) assures medical care by providers familiar with the workers’ compensation system. Distribute this brochure to existing employees prior to implementation of the approved MPN. Also, provide to each new employee at the time of hire and to each injured employee at the time of injury:
